I do hereby consent to authorize and direct chaperones of Community Baptist Church to obtain for my child such medical care, treatment, and hospitalization as may deem necessary. I do hereby release, remiss, and forever discharge the chaperones and Community Baptist Church for any and all claim, demands, actions or cause actions, past, present, and future, known or unknown, arising out of any injury to my child. I give permission to Community Baptist Church to take photographs and/or videos of my child which may be used for internal publication and/or associated social media.

RELEASE AND WAIVER OF LIABILITY

Event:  Community Baptist Church, Spring Break Camp 

1.         Inherent Risks: There are risks, both known and unknown, using any inflatable or amusement device, or other amusement or sporting activities, or swimming pools including, but not limited to physical injury, emotional injury, distress, paralysis and even death. The risk of serious injury from participating in inflatable bounce house activities and pools, although minimal, does exist, as it does in all play and amusement activities. 

2.         Supervision: Parents/Camp Director/Camp Leaders will need to remain with their children at all times while inflatable bounce houses are being used. Parents will remind children to follow all rules of participation and will trust their judgment or the judgment of church representatives in their safe participation. Church does not supervise your children and is not responsible for their safe participation, the parent or guardian is responsible for supervision of themselves and their mnors.

3.         Assumption of Risk: The undersigned hereby acknowledge and agree that they understand the nature of the Event; that Participant is qualified, in good health, and in proper physical condition to participate therein; that there are certain inherent risks and dangers associated with the Event; and that, except as expressly set forth herein, they, knowingly and voluntarily, accept, and assume responsibility for, each of these risks and dangers, and all other risks and dangers that could arise out of, or occur during, Participant’s participation in the Event.

4.         Release and Waiver: The undersigned hereby RELEASE, WAIVE, INDEMNIFY, DISCHARGE AND COVENANT NOT TO SUE Community Baptist Church and its officers, volunteers, agents, and employees, (collectively, the “Releasees”), from any liability resulting from any personal injury, accident or illness (including death), and/or property loss, however caused, arising from, or in any way related to, Participant’s participation in the Event, except for those caused by the willful misconduct, gross negligence or intentional torts of the above parties, as applicable.

5.         Indemnification and Hold Harmless: The undersigned also hereby agree to INDEMNIFY, DEFEND AND HOLD the Releasees HARMLESS from any and all claims, actions, suits, procedures, costs, expenses, damages and liabilities including, but not limited to, attorney’s fees, arising from, or in any way related to, Participant’s participation in the Event.
